Here are the forms you need for an application for an entry visa or a residence permit for visits to Sweden and the appendixes and questionnaires that you must fill in.
The forms are pdf files and open up in new windows. You can fill in the form in your computer and then print it out and sign it.
Application for Schengen Visa

A visa is a permit issued for stays of short duration.
National visa (D-visa)
Form no. 169011
Use this form if you are outside Sweden and want to apply for a visit to Sweden for more than three months and up to one year. Submit your application to a Swedish authority abroad (embassy, consulate or equivalent).

You must here list your (the applicant´s) parents, husband/wife/partner, children and siblings. This form must also be filled in if you are applying for an extension.
Invitation - If you wish to invite a relative or friend to visit you

If you wish to invite a relative or friend to visit you, you must fill in this form.
Application for residence permit for a child who has been, or will be, adopted
Swedish form no. 170011

Use this form to apply for a permit for a child under the age of 18 who is to be, or has been, adopted, and where the adoption is not negotiated by an authorized Swedish adoption organisation.
Questionnaire concerning a person who wishes to visit you in Sweden

Only use in this form if you have received a request from the Migration Board to fill in this form because a person has applied to visit you.
