Here are the forms you need if you wish to apply for a residence permit in order to move to a close family member in Sweden.
On the pages for different types of permits you can see which form you need and which attachments you must append to your application.

The forms are pdf files and open up in new windows. You can fill in the form in your computer and then print it out and sign it.

For EU/EEA citizens and their accompanying family members there are specials forms, see link in the column to the right.

Residence permit to settle in Sweden
Swedish form 160011
English form 161011.

Use this form if you want to apply for a permit to settle in Sweden. You must also use this form when applying for an extension of your residence permit. (NOTE! EU/EEA citizens, relatives of Swiss citizens, long term resident in another EU/EEA country and their relatives must use a
different form.)

Replaces the form no. 115 021.

Residence permit to settle in Sweden – for a child under the age of 18
Swedish form 162011
English form 163011.

Use this form to apply for a residence permit for a child under 18 years to settle in Sweden. You must also use this form when applying for an extension of a residence permit. (NOTE! EU/EEA citizens, relatives of Swiss citizens, long time residents in another EU/EEA country and their relatives, must use a different form.)

Replaces the form no. 113 021.

Assurance of cohabitation for married and common law spouses
Form no. 228011

Questions to the referee in Sweden who is married or is common law spouse with someone residing abroad who has applied for residence permit in Sweden. NOTE! Only to be filled in by persons who has received a letter by the Migration Board to do so. Please do not submit the form before you get the letter. Only in Swedish.

ONLY TO BE USED WHEN APPLYING AT THE SWEDISH MISSION IN WASHINGTON, BELGRADE, NEW DELHI AND MOSCOW. This form should be used by persons applying for a residence permit who are married or cohabiting with a person living in Sweden.

Form for minors with parents in Sweden
Form no. T93

ONLY TO BE USED WHEN APPLYING AT THE SWEDISH MISSION IN WASHINGTON, BELGRADE, NEW DELHI AND MOSCOW. This form should be used when one or more minors are applying for a residence permit and wish to take up residence with a parent living in Sweden.

Form for persons who are planning to marry or cohabit as well as their accompanying children

ONLY TO BE USED WHEN APPLYING AT THE SWEDISH MISSION IN WASHINGTON, BELGRADE, NEW DELHI AND MOSCOW.  This form should be used by persons applying for a residence permit who plan to marry/cohabit with a person living in Sweden. Also for accompanying children.

Form no. T84

ONLY TO BE USED WHEN APPLYING AT THE SWEDISH MISSION IN WASHINGTON, BELGRADE, NEW DELHI AND MOSCOW. This form should be used by persons who are applying for permission to reside with a person who is living in Sweden (reference person).
