Here are the forms for EU/EEA citizens and their close relatives, for Swiss citizens and persons who are long-term resident in an EU country.

The forms are pdf files and open up in new windows. You can fill in the form in  your computer and then print it out and sign it.

Registration of right of residence

If you are a citizen of an EU/EEA-state, you should use this form to register your right of residence. When you have been registered, you will receive a certificate that proves your right to reside in Sweden.

Form no. 142011

Application for certificate of permanent right of residence and application for permanent residence card. To be completed after five years of residence in Sweden by persons who are an EU/EEA citizen or family members of an EU/EEA citizen but not are EU/EEA citizens themselves. Only in Swedish.
