Questionnaires for the relative in Sweden

If your family member applies for a residence permit at an embassy or consulate-general you will receive a letter asking you to download a questionnaire.

In the letter you will be given information about which questionnaire to fill in. You should not fill in a questionnaire before you have received instructions on which form to fill in.

Each questionnaire opens as a PDF in a new window. Fill out the questionnaire on your computer and print it out. Don't forget to sign your name before submitting it.

If you don't have a printer, your local library or citizens advice bureau may be able to print it out for you.

  • Assurance of cohabitation when applying for extension of a residence permit

    If you apply for an extended residence permit you must fill in this assurance together with your husband, wife, cohabiting partner or registered partner. This is to prove that you are still living together. Fill in the form, print it and sign it. Please note that both of you have to sign it.
